The last thing you want to do is consolidate your debts.  All that does is cost you more money in the long run.  They charge higher interest, usually.  Even if you went to your bank and got a home equity loan, you'd still end up paying more than necessary in the long run... and worst of all, you will still be in debt, because you could run up more credit card debt while you're trying to pay that off.

The best thing to do, if you want to get out of debt, and stay out of it, is go to Consumer Credit Counseling.  They are a non-profit organization, and they are members of the Better Business Bureau.  They will not only counsel you on how to stay out of debt, they will take all your monthly statements, contact all the creditors and get them to reduce your interest, and waive all late fees.  The interest rate goes WAY down.  You make one monthly payment to them, they charge a very small monthly fee (about $25) and the best thing is that your payments are on time.  Depending on how much debt you have, they can settle it all for you in 3-5 years.  You have to cancel all your credit card accounts to get in the program, but it is SO worth it.  Living without credit cards is the best choice there is.  Once they are all paid off, your credit rating will be higher and then you might want to get just one card, to make it easier to rent cars and book flights, etc.
